(c) Authority Letter to Lead Litigant to Sign Affadavit

Kindly fill up your personal particulars, sign and leave all other particulars blank to be filled up by TSEWA. Pin the above documents in the same order and attach fifth document which should be your PPO attested by yourself. Attach Rs. 5,000/- crossed cheque (At Par) / DD drawn in favour of TSEWA-AFT. Please write your TSEWA Membership No. in Pencil on top right hand corner.

If you are a non Member of TSEWA, it is incumbent to become a Member to meet the legal requirements. The application form is available for download.

Kindly complete the form carefully (Name as in PPO) and send a separate crossed cheque (At Par) / DD for the Membership fee as applicable plus donation (Optional) in favour of TSEWA, Account No. 35088276014.
